Es un gusto después de varios meses poder estar escribiendo para ustedes en esta hermosa comunidad de ciencias, mis queridos amigos en esta oportunidad quisiera hablarle de una lesión llamada granuloma piógeno, para que en caso de que puedan tener esta lesión puedan asistir al odontólogo, paciente femenina de 72 años de edad asiste a consulta odontológica por presentar inflamación y dolor en el cuadrante superior izquierdo manifestando estar preocupada por dicha lesión al observar la clinicamente se pudo evidenciar que la paciente presenta diversos restos radiculares que son pocos infecciosos para la cavidad oral.

It is a pleasure after several months to be writing for you in this beautiful community of sciences, my dear friends in this opportunity I would like to talk about a lesion called pyogenic granuloma, so that in case you may have this lesion you can go to the dentist, a 72 year old female patient attends dental consultation for presenting inflammation and pain in the left upper quadrant expressing to be concerned about this injury to observe the clinically it could be seen that the patient has several root debris that are few infectious to the oral cavity.

Cabe destacar que mediante el examen clínico oral no se establece un diagnóstico definitivo de la posible lesión, pero este sí nos puede orientar a tener diagnósticos diferenciales de posibles lesiones ya que existen gran cantidad de diferente tipos de lesiones, que pueden estar presentes en las personas, para lograr tener un diagnóstico certero de la lesión se debe realizar un estudio histopatológico, tomando una muestra de la lesión y esto lo debe hacer cirugía bucal para sí tener el comportamiento adecuado para tratar las lesiones y el tratamiento quirúrgico más idóneo o certero.

Al comenzar la inspección de la cavidad bucal y observar la lesión en la paciente pudimos evidenciar que la paciente presenta una área enrojecida, eritematosa, violacia con inflamación y sobreelevada de superficie brillante, con un diametro aproximando de 3 centimetros, localizada justamente en el vestibulo superior izquierdo, la paciente no presentaba radiografícamente panorámica dental por lo que era mucho mas complicado poder observar mas alla dos hallazgos clínicos, pero se le indicó mandar hacer.

Pero ahora bien conozcamos un poco más acerca del granuloma Piógeno este tipo de lesión es un crecimiento de carácter no neoplásico, es decir que no representa un riesgo de malignidad, suele desarrollarse como una respuesta exagerada frente a un trauma, esta lesión etiológicamente se produce como una respuesta a una irritación o trauma, es considerada como una lesión reactiva o del grupo de lesiones hemorrágicas tiende a hacer una lesión sobreelevada, que presenta superficie lisa o un lóbulada en ella no hay secreción purulenta.

But now let's know a little more about Pyogenic granuloma, this type of lesion is a non-neoplastic growth, i.e. it does not represent a risk of malignancy, usually develops as an exaggerated response to trauma, this injury etiologically occurs as a response to irritation or trauma, is considered as a reactive lesion or group of hemorrhagic lesions tends to make a raised lesion, which presents smooth surface or a lobulated in it there is no purulent discharge.

En cuanto a la localización que posee es mucho más frecuente que podamos encontrarla en el maxilar superior, que en la mandíbula, siendo más predominante en anterior que en posterior, es muy común que se encuentre en la encía marginal y papilar, como podemos evidenciarla en este caso también es una lesión que puede encontrarse en el labio, en la punta de la lengua y así también como en la mejilla en algunos casos esta lesión puede sangrar, puede ser asintomática pero también se puede presentar dolor, en cuanto al color se puede presentar rojo brillante y puede tener tejido ulceración suele ser una lesión más roja, tiende a sangrar cuando está reciente a medida, que tiene tiempo se vuelve rosáceo por lo que se puede determinar que esta lesión tiene tiempo en la paciente.

As for the location it has is much more frequent that we can find it in the upper jaw, than in the jaw, being more predominant in anterior than posterior, it is very common to be found in the marginal and papillary gum, as we can see in this case it is also a lesion that can be found in the lip, in the tip of the tongue and also in the cheek in some cases this lesion can bleed, can be asymptomatic but can also present pain, in terms of color can be bright red and may have ulceration tissue is usually a redder lesion, tends to bleed when it is recent as it has time becomes pinkish so it can be determined that this lesion has time in the patient.

La literatura expresa que es más común que la encontremos el niño y en jóvenes, pero puede aparecer en cualquier edad, en cuanto a su textura es lisa y brillante, su consistencia tiende a ser lobulada su tamaño puede ser de menos de 2 mm hasta poder alcanzar 5 cm, en esta lesión existe una gran proliferación de tejido de granulación que produce mucha vascularidad, al ser una lesión de tejido blando, no debe aparecer en una radiografía panorámica, es mucho más común, que aparezca en mujeres embarazada debido a los altos niveles de hormonas que posee y en esos casos va a recibir el nombre de tumor del embarazo.

Por último el tratamiento de esta lesión es realizar una incisión que pueda llegar hasta el periostio de la encía haciendo así un buen curetaje, eliminando la lesión para evitar que esta pueda tener alguna recidiva, justamente después de eliminarlo se debe realizar una tartrectomía bien profunda.

Finally, the treatment of this lesion is to make an incision that can reach the periosteum of the gum, thus making a good curettage, eliminating the lesion to prevent any recurrence, just after eliminating it, a deep tartrectomy should be performed.
